Required Forms for Sprucelands Camp

These forms are necessary for your child's participation in Sprucelands Summer Camp.

All forms must be signed by you the parent, and several forms also require the signature and commitment of your camper. We recommend that you talk over the meaning of these forms with your son or/and daughter.

By making the forms available to you online, we save your having to wait for the mail. Should you have difficulty downloading these forms, contact us immediately and we will be happy to send them out to you.

Medical Form

The medical form should be brought with you to camp on your Opening Day. Do not mail it ahead of time. And make a note that the medical form must be redone annually per the request of the local Health Department.

Horse Application

There is a form available for those choosing to bring their horse to camp. It gives our barn manager detailed information about how you perceive your horse, what your horse's habits might be, where your horse fits in the herd, and whether your horse manages the pasture well.
